Permanent Supportive Housing

Prairiewood Permanent Supportive Housing Program for the Chronic and Long-Term homeless has 4 units of townhomes in Faribault, MN.  Persons must meet the HUD definition of Chronically homeless which is that they are continuously homeless as defined by HUD for 1 year immediately prior to entering the program or have had 4 distinct episodes of homelessness as defined by HUD in the past 3 years, which add up to one year of homelessness, and they must have a documented disability.  Persons qualifying for the program will receive rental assistance based on 30% of their adjusted gross income and will be offered on-going supportive services to assist with housing stability.  For questions about the program, please contact 800-277-8418.  Applicants will be accepted from the Coordinated Entry System list.  Please see Support Services Tab.  Persons who are already on the list do not need to make a repeat contact.
